Purpose: The “Organization Info Form” is a fillable single-page document that will contain your mission statement, organizational bullet points (population you serve, key activities, major accomplishments), and your potential project idea. This document will facilitate your communications with funders, both by phone and email. It can also serve as a useful internal tool, to help identify other potential grantmakers whose values and funding priorities are a strong match with those of your organization.
